The service scopes involve all inquiries relating to application and modification of password or other personal ID.

l      Application/ Modification Form for Password and Personal ID

In case of a forgotten password, fill out and submit the above form along with a copy of personal identification document.

l      Application Form for Email Account (for faculty staff)

For those who are first-time applicants for email accounts of the University, one should complete and submit the above form to the Computer Center before they can be assigned with their individual email accounts.
